Title IX Reporting Form

To Report an Emergency or if you are in immediate danger, call the Lasell University Campus Police at 617-243-2269 or 911 before completing this form.

This form should be used to report possible violations of the University’s Title IX Sexual Harassment Policy or the University’s Sexual Misconduct Policy. The full text of both policies can be found here:

Filing a complaint of sexual misconduct serves as notice to the University and authorizes the Title IX Coordinator to take preliminary action to investigate the information/allegations contained within the report. For questions about the University’s response to sexual misconduct, contact the Title IX Coordinator, Maryrose at 617-243-2270 or manthes@lasell.edu. You may also contact her directly to report an incident.  All reasonable efforts will be made to maintain the involved parties’ privacy during the investigation and resolution procedure.  It is unlawful to retaliate against any student, employee, or any other person affiliated with the University for filing a complaint or for cooperating in an investigation or complaint.  Submitting this form will trigger outreach and contact by our Title IX Coordinator Maryrose Anthes to the reporting party and impacted party.

You may file this report anonymously by omitting your name and contact information from this form. However, depending on the circumstances and kind of information disclosed anonymously, it may not be possible to investigate or provide services to an undisclosed victim and the University’s response may be limited.

While we encourage students, employees, and others to report incidents of sexual misconduct so that we can work to enhance the safety of our community, hold perpetrators accountable and render appropriate accommodations to survivors, we also understand the challenges and difficulties such processes can entail. If you are unsure if you want to make this report or need more information before doing so, Lasell offers confidential resources on campus for students through our Counseling Department and Health Services. Students and employees may also contact a Confidential Resource Advisor on campus. Off-campus organizations are free and confidential and are available to students and employees. Discussions with these programs will not trigger a complaint or investigation and will give you the time and space you need to make a decision that works best for you.
